Position Description:
GENERAL SUMMARY

Responsible for coordinating, negotiating, obtaining services and resources to facilitate meeting the patients’ health and human service’s needs.

KEY RESPONSIBILITIES

1. Identifies and understands what resources are available and builds and maintains effective working relationships with a network of interdisciplinary teams within CGMHSA and the community.
2. Advocate for patients to facilitate positive outcomes.
3. Minimize fragmentation of care within CGMHSA and with facility partners.
4. Identify tools to improve adherence to the plan of care for the patient, including medication adherence and internal/external appointments compliance.
5. Promote the use of evidence-based care, as available.
6. Assist the patient with navigating CGMHSA and its partner facilities to achieve successful care.
7. Conduct assessments of the patient’s health and psychosocial needs, including health literacy status and deficits.
8. Encourage appropriate use of health care services, strive to improve quality of care, and maintain cost-effectiveness on a case-by-case basis.  
9. Utilizing the facility’s EHR, perform documentation of the patient’s assessment, interventions, and ongoing collaboration with the patient, family/caregiver, providers, and other pertinent stakeholders.
10. Displays effective communication skills; adheres to proper communication channels in a courteous, cooperative, and timely manner.
11. Demonstrates personal accountability for professional growth by meeting and maintaining current certifications/licensure and completing all mandatory facility, department, and role-specific education requirements in advance of due dates.
12. Consistently protects patients’ rights and confidentiality.
13. Supports quality and fiscal goals for the department and institution, assists in inventory management, assists with CQI projects, and identifies obstacles to improvement within the clinical area, assists with policies and procedures development for the facility.
14. Serve as a peer mentor and provide student learning experiences as assigned.


15. Participates in special projects that supports the operation of the organization and other duties as assigned.

Position Requirements:
MINIMUM QUALIFICATIONS

• Graduate from an accredited Registered Nursing (RN) program is required. Three years of clinical nursing experience. Work experience may NOT substitute for education requirements. Strongly Preferred: Three years of case management experience; Case Management Certification.
• Currently licensed or licensed by endorsement as an RN by the Alabama Board of Nursing.
• Must possess basic knowledge of computers, PC applications, software, and database management.
• Current CPR certification from the American Heart Association is required.

WORK ENVIRONMENT

This role may require lifting up to 50 lbs.
The work location for this role is onsite.
