PRIME Education, Pack Health, and Brookwood Baptist Health are engaging in collaborative quality improvement programs to advance shared decision-making between patients with type 2 diabetes or chronic obstructive pulmonary disease (COPD) and their health care providers.
"We are pleased to be a part of this study which will assist people in our community living with diabetes or COPD," says Dr. Anne-Laura Cook, Medical Director of the Population Health Management and Primary Care Innovation at Brookwood Baptist Health. "With this partnership, our physicians and clinicians will be able to gain insights into their patients' experiences with their care while working with patients to improve the management of their chronic conditions."
Brookwood Baptist Health primary care providers and their patients will be able to participate in this free study supported through educational grants from Novo Nordisk, Inc. and AstraZeneca. The programs combine objective patient data, subjective survey data, and accredited educational interventions to teach doctors, other clinicians, their patients and families how to work together to improve outcomes of health.
